Registration Forms
REQUIREMENTS FOR ALL ATHLETES
Each student athlete is required to set up a DragonFly account prior to participation.
DragonFly requires the following forms to be uploaded:
- Birth Certificate
- Physical
- Insurance Card
In addition, ALL student athletes are required to fill out the following forms and return them to their coach BEFORE participating in any practice or game.
